If you have an accident or near miss while performing your duties as a franchisee, we require you to fill in and send the attached Hazard Report Form.

If you observe a potential hazard, or something that you believe the franchisor should warn other franchisees about, please also fill out and send the attached Hazard Report Form.

We will use these reports to assist us in improving our Health and Safety.
